The purpose of the B-3 Early Childhood Special Education Needs Teacher is to provide Early Intervention and Service Coordination services to children and families referred to the Birth to Three Program for the Sauk County Department of Human Services.

Essential Duties
Receive referrals for Sauk County’s Birth to Three Program; complete developmental screen and Family Directed Assessment with family to identify areas of concern and to help with outcome development; complete Routines Based Interview with family; obtain required parental consent; and inform parents regarding their rights in the program.

Participates in team development of the Individualized Family Services Plan (IFSP).

Provides intervention services with child and caretaker to help meet IFSP outcomes in natural environments utilizing Primary Coach Approach evidence based practices.

Provide overall coordination of services as the Service Coordinator on selected teams.

Completes formal evaluation of developmental skills to determine eligibility.

Complete and submit six month and annual reviews of progress towards IFSP outcomes.

Communicate and coordinate services with medical professionals, school personnel and other community resource providers.

Maintain a strong connection with school personnel and Head Start staff to promote/ provide seamless transition services for children with special needs.

Responsible for child find, community awareness, parent/family networking activities and educational activities for families and the community.

Assist in file reviews and self-assessment process.

Enter information into electronic systems for billing, documentation and state reporting.

Provide unit case management in absence of assigned Service Coordinator/Social Worker.

Completes necessary documentation and paperwork to meet the federal, state and local birth to three standards for insurance revenue purposes.

Run monthly Datamart reports and report to supervisor and staff on findings.

Participate in program development, guidelines, and explore materials that will help improve program.

Order and maintain Birth to Three equipment and materials.

Arrange or provide client transportation for medical/Birth to Three services.

And other duties as assigned.

Required Working Hours

Standard working hours are Monday through Friday, 8:00 am until 4:30 pm. Please note these hours are subject to change and additional hours may be needed or required. Compensation for additional hours beyond normal working hours are subject to applicable state and federal regulations.

Minimum Training and Education
Required:

Bachelor’s degree in Early Childhood Special Education

1 year of experience working with families with special needs

Licenses/Certifications:

Licensed Teacher through the Department of Public Instruction under 808 or 809

Valid Wisconsin State Motor Vehicle Operator’s License

Preferred:

First Aid/Emergency training certified

Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
